Join the HEAT Swim Team

Please print out and complete the following registration forms. Download and print out forms by clicking on highlighted links:
- 2010 USA Registration form
- A COPY of the child's birth certificate.
- * Annual HEAT team fee of $75 individual, $100 for more than one swimmer, due upon joining team or annually Sept. 1
- Payment of first month’s tuition is due upon joining. If start date is after the 15th tuition will be one half the monthly rate. For monthly dues fees see dues information review the terms and conditions contract.
- HEAT Registration Forms
Complete all forms and bring to your first practice. Please make one check payable to HEAT for all of the fees. Monthly dues will be automatically deducted from your checking or savings account. Completion of the auto debit form is required when joining the team.
Due to liability it is our policy to have all registration paperwork prior to starting the team.
* Team fee is reduced by one half May-August then payable annually September 1.
